What a good decision record looks like, and why ours are dated

A decision without a date and evidence is an opinion. Here is the shape we use.

Five fields, every time

Every DevRamp decision has a statement, a reason, linked evidence, an owner and a date. That is enough for a person or an agent to trust it, or to know when it is stale.

Why dates matter

Decisions change. Retries went from 3 to 5 to 7. A dated record lets the newest call supersede the old one, so nobody acts on a decision the team already reversed.

Why evidence matters

An answer that cites PR #412, LIN-238 and the #payments thread can be checked in seconds. One that cites nothing cannot.
